Send your referral letter to Mindler

If you want to be reimbursed for your treatment at Mindler through your health insurance, you need a valid referral from your general practitioner. To get a referral, you make a (digital) appointment with your doctor. Then your GP sends the referral digitally via Zorgdomein to Mindler, or creates it on paper.

Do you have a referral letter on paper? Download our app on your phone or tablet and create an account. Then scan or take a photo of your referral letter and upload it below. We will connect your referral to your account.

Your referral is only valid if:

  • It says you’re being referred to Mindler
  • You are referred for Basic GGZ, and not Specialized GGZ
  • Your name, date of birth, address and BSN number on it
  • The referral is no more than 9 months old
  • Your name, profession and AGB code of the referrer are listed
  • The referrer’s signature and/or the practice stamp are included in the referral letter
  • The reason for referral has been stated.

Note: If your referral letter does not meet the above requirements, your booking with Mindler may be cancelled.
